Well Child

Children should have a Well Child Physical at:

1 month,
2 months,
4 months,
6 months,
9 months,
12 months,
15 months,
18 months,
2 years,
3 years,
4 years,
5 years,
Children over 5 years of age should have a physical every 2 years

Physicals Include

General assessment,
lead screenings (ages 1-6 years),
height & weight,
complete physical,
vision screening,
hearing test,
immunizations,
nutritional education,
and other consultation as necessary.

Physicals & Immunizations: $35.00

Fee birth to 18 years:

Physical Only: $25.00
Immunizations: $15.00

Referrals can be made to other needed services including WIC, Child Health Specialty Clinics, AEA 267, Early Access, Parents as Teachers.
