Well Child
Children should have a Well Child Physical at:
- 1 month,
- 2 months,
- 4 months,
- 6 months,
- 9 months,
- 12 months,
- 15 months,
- 18 months,
- 2 years,
- 3 years,
- 4 years,
- 5 years,
- Children over 5 years of age should have a physical every 2 years
Physicals Include
- General assessment,
- lead screenings (ages 1-6 years),
- height & weight,
- complete physical,
- vision screening,
- hearing test,
- immunizations,
- nutritional education,
- and other consultation as necessary.
Physicals & Immunizations: $35.00
Fee birth to 18 years:
- Physical Only: $25.00
- Immunizations: $15.00
Referrals can be made to other needed services including WIC, Child Health Specialty Clinics, AEA 267, Early Access, Parents as Teachers.
